Are you referring to this form http://www.jotform.us/form/70335894910157? If yes, I have cloned your form to check the Smooth Signature widget properly. I did a test submission and I was not able to replicate the issue. Here's the notification email screenshot:
Are you still having an issue? If yes, could you please try opening your email in a different internet browser? I used Google Chrome and Gmail account for the email recipient.
To check the Smooth Signature if it is working or not, please fill up the form manually.
