Form not working from Mobile Devices

    We are using your service on our website however when someone pulls up the website from their mobile device they are not able to see the form.  Can you please assist.

    I checked your web page on my mobile phone and I can see the form is showing. Here's a screenshot from my mobile:

    I suggest you insert the Mobile Responsive widget to your form. Here's a guide on How-to-Add-a-Widget-to-your-Form.
