submission information missing question from form

Have a form which looks nice on website,

But when looking at submission information all I get are the inital - signature widget data.  How do I get the text field and the input field, signature app, both on the submission report?  I have also included the webpage that has the link.

You need to mark those fields as required.

So first select the field. Click on the properties (blue gear icon) 

Do this for each field
