I checked your form and I found issue with the submission spreadsheet.
It seems the integration was broken due to edition done on the form.
I suggest you redo the google spreadsheet integration on your form at this time.
See if this process resolve the issue.
Let us know if issue persist.

I apologize for the confusion. I was referring to the Sheet not the form.
I meant the Sheet had been edited or modified in the Google spreadsheet used in the form integration.
Try redoing the integration by removing it and then add it again.
Let us know if issue still persist even after doing this.

The integrated sheet should not be edited. Rearranging the columns will indeed break the integration. If you would like a sheet that can be edited, I would recommend creating a new sheet and using the import range function to import the data from the integrated sheet:
