Hello Terry,
This is very strange and very unlikely to happen. If you type in a valid usernam/email & password, there is no reason that login will fail.
Are you sure you are entering correct password to sign in to your JotForm account? Have you shared your account password with someone else too? I did check your account history and found that found that one IP is used to login to JotForm account regularly but there are two other IP addresses as well which is being used to login. Please check the screenshot below:
I would suggest you to please change your account password again and also change your profile email address as well to ensure no one has access to your account.

Hello Ashwin
You are correct; at one stage I did share the login with a team member so he could modify a form that he had worked on.
I now have the situation that JotForm is telling me I have reached the limit for login attempts, and has locked me out. Where should I go from here?
I can't change the email address in the profile, as it is my personal address within the company, and I have no other suitable company address I can use.
