Printing a certificate of completion after completing an evaluation form

This is a probably a form that is three-parter

a. Demographics - name, address,  license number, and other credentials

b. An evaluation questioner  (can this be linked  in survey monkey?). After completing this surgery monkey which may contain fields of sessions in a conference attended.

c. A complete certificate template that contains most of (a) information; sessions attended in survey monkey form and this is printable

You can do this with Jotform. However, I don't think you can connect our forms with another form builder without using integration. One example is through Zapier: https://zapier.com/zapbook/jotform/surveymonkey/.

Note that you can also utilize our auto responder feature to create the certificate for printing.

In the autoresponder email, you can add the field data to personalize the certificate.
