Why is my account suspended???

I have violated no data collection rules. Why is my account suspended????????

I see that the account was suspended due to the form http://www.jotform.us/form/70117796278163 in your account. It looks like that you have deleted the form and still existing in the Trash folder. Unfortunately, it is not allowed to collect credit card information on plain text fields using JotForm. Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.  

I have now activated your account and you should be able to login to your account. Please remove the guilty fields from the form immediately from the form to avoid any further suspensions. If you do not want to use that form, you may delete it permanently from the Trash folder. 

I can not see http://www.jotform.us/form/70117796278163 in order to remove it. I see nothing to delete.

As your account no longer have the form ID 70117796278163, you do not need to perform any further actions and all you need to do in the future is to keep your forms in compliance with our terms of use as mentioned by my colleague Kiran in his previous reply.
