I have tried submitting your JotForm on the web page and see that the blank page is displayed. However, as I tried to submit the cloned form in my account, I was able to see that Thank you page is displayed correctly.
Could you update the Thank you page once and see if it is displaying correctly after submitting the form? Also, please try embedding the form using iframe embed method and check if that works.

I have tried cloning the form again to my account and was unable to replicate the issue at my end. However, it seems to be the issue occurring on your form itself. I have also tried editing the Thank you page on your form which didn't work. Do you have any integrations on the form? If there are any, could you try removing the integration once on the form and then submit the form to check if that works?
