my jot form is not working and is costing us lots of $$$$ Please fix ASAP - link provided below

    Our jot form links are not working and is preventing us form booking work and completing sales. the links for our jot forms are below. Please fix this ASAP as there is financial hardship involved. Thanks

    We do not allow for the collection of credit card details through forms.  As per our terms of use:

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    Your "Agreement for Moving Services form requested such data.

    We have recently needed to become much strict on this due to becoming PCI compliant.  This required removing any currently stored credit card data from our system.

    If you require payment authorization, that can be done through one of our many payment processors:

    If it is OK with you, we can remove the credit card fields from the form and activate your account.

    OK, we will have to find another solution to this issue. In the meantime, if you could remove the credit card fields, replace it with the following paragraph and activate our account that would be helpful. Thank you.

     

    Reliable Delivery requires a valid credit card number to hold your reservation. If you cannot keep your reservation, a 72-hour change or cancellation notice is required. All reservations cancelled within 72-hours of your scheduled move date will be charged a $100 cancellation fee. We will contact you shortly for your credit card information.
