"Please Wait" when applying

Hello there,

Someone trying to fill out our form: www.jackalopeartfair.com/applypasadena is getting a "Please wait" when trying to submit their form.  I tried and also got this note.  Seems like the form is held up for some reason?  Can you please help me relieve this?  I just sent an email blast directing people to this form and really need it to work ASAP.
