Can I create a form for members to verify their current information?

I have a list of committee members who are are multiple committees.  I'd like to send them a form asking 1) if they'd like to opt back into my committee list.  If they say yes, then 2) have them verify their contact info (phone number and address) that I hope can be uploaded to the form for them to review/change.
