Could you try adding the logo image to the image section of the header and see if that makes any difference while sharing the form on the Facebook?

I tried the above suggestion on getting my image to show up but it still is not there. Other ideas?
The presence of our logo or a picture on the Facebook page attracts more attention as people scroll through the posts so that they will stop and read and complete the JotForm.

I can only assume that you have previously shared your form in Facebook when it still doesn't have a logo. And since Facebook saves the information of that link you shared previously, even if you add a logo to the form, it will still recognize the old details of the link.
To fix the issue, I have used the Sharing Debugger Tool on your form link to scrape the old information and update it with a new one.
You should be able to see the logo of your form after you share it again.
