I have a question about the language settings.
It seems when i go via us proxy that the form
is still in dutch.
Can you check this out for me?

The form detects the default browser language, not the users current location. When I load the form on my end, it is in English by default. It looks like some of the changes may have been saved under English instead of Dutch due to the default language being Dutch when the payment field was added.
If you clone this copy of the form:
Then add Dutch in as the alternate language, it should then work properly.
