Why does my custom form link not work?

I created my t-shirt order form and I'm ready to publish and share. When I try to preview, it just pulls up a blank screen. I published anyway and tried to use the live link and it was still just showing the blank screen. Can you tell me what I need to do to get this going?

It looks like that you have two forms:

1) First with configured custom URL:

2) Second without configured custom URL:

https://form.jotform.us/72185170987163
