Thank you. For some reason I still can not view the form. I've tried to open it from multiple devices and in different browsers (Safari and Chrome) but still have not been able to gain access. 

I checked your form and I got the same result as my colleague. The form is working fine on our end. Please try opening this link (www.jotform.com/form/72194989213163) and see if that works for you.
