How can I change the name of my form?

    Under MY FORMS I have an AZ Lottery Recap form. It say AZ Lottery everywhere when I open it. Yet, when the confirmation email comes to me that there is a submissions is says RE: AZ LOTTERY RECAP and then 

      New Amsterdam Recap Form

    How Can I change this to say AZ LOTTERY?

    That's because your form title and title remained as old one.

    So to change that please follow:

    1) Go Settings - Form Settings and change the Title:

    2) Scroll a bit down and change Form Title:

    The Page Title does correctly say AZ Lottery Recap Form. But when I get the email it says New Belgium still. hum...

    Hmm, it should be saying "New Amsterdam Recap Form" according to my tests, and that is because, in the email notification content, and right before the table with the "Name", "Date" and all the remaining fields, you have a row that has "New Amsterdam Recap Form" and changing it should solve your issue.

    Please note that this row is not visible when you open the notification, because you need to scroll to the right in order to see it. Here is a screenshot to show you how I found it:

    Oh man! I tried and just sent a test but it did not work. I would love it if you can help. I give you full permission to access our Jotform and change it to say "Arizona Lottery Recap Form"
