I would like to add an auto-generated number each time my form is accessed (so the inputter can cross reference the online form to a paper record).
Is this possible please? Thank you so much :) Laura

Hello lauraNTU,
If I understand your question correctly, you need a auto-generated number every time form is accessed. Is that correct?
Yes it is possible to achieve your requirement by adding "Unique ID" widget in your form. Please note that the widget will auto-generate a unique ID every time the form is submitted but not on just accessing the form. You may like to take a look at the following guide which should help you: https://www.jotform.com/help/79-How-to-Add-Custom-Unique-IDs-to-your-Form-Submissions
Hope this helps.

Thank you for your answer, this is really useful.
Is possible to generate a unique ID that the customer/inputter can see? If not, I think I have worked out how to get what I need by using a condition and other fields in the form.
thanks again

Hello Laura,
Please note that unique id is generated only after the form is submitted and that it the reason why it is not possible to display it in form before the form is submitted.
It will be added in the notification email alerts and you can also display it in thank you message which is displayed after form is submitted. Please check the screenshot below how to display unique id in thank you message:
Hope this helps.
