1.i am getting the submission twice in my accunt, means all the forms r getting in my email twice.
2. my customers are also getting the same form, as i am getting in my email with there information

I cloned your form and I found out that you have 2 email notifications. This means that when someone completed your form, you will receive 2 email alerts. Please delete the other notification so that you will only receive 1 email alert. Here's how:
In Form Builder, click "Settings" in the top menu and then go to "Emails" section. Select the notification and then click the "Trash" icon.
If you don't want your users to receive an auto-reply or confirmation email, then please delete the autoresponder in your form. Here's how:
