It looks like its already doing that. Its on the left side. And it allows you to scroll up to the top at any stage.
Please tell me what exactly did you want it to do?

I see that when I am previewing the form via JotForm, but when it's on my website, it's on the very bottom of the form only.

Lets try using the iFrame embed method. This will ensure the embedded form will behave identical to the stand alone form:

However, I have conditions set under "Position Status". The application with the conditions work if I use the EMBED option. But when I embed the Full Source Code, the options for Position Status are all hidden. (See below)

May you please embed the source code on your webpage and check if the issue persists?
If so, please let us know and keep the source code embed method, we will check it and find why the fields are not displaying on your end.
