Form has wrong title in the notification email

Hi, I have a form I am using that I have titled GBFC Client Profile.  However, when the client submits it and it comes back to me it is saying "Midas Financial Client Profile" (my previous company name).  It wasn't doing this but then all of the sudden started to. I put the link to the correct form in the url space and then a screenshot of how it comes to me in the screenshot space.

Please advise!

I've fixed it for you. All you need to do is to change that in the notification email template. You can use the title tag or type the form title manually.
