On our "Contact Our Companions" form, the top eight areas of interest are not forwarding to the designated e-mail address, email@example.com. The bottom two, which are forwarded to a different e-mail address, are working. Our IT company checked and said that the above e-mail address is working fine, so there appears to be a problem with the form.

I have checked the email addresses related to your form and noticed one of them is in our bounce list, this is the reason:Result: h******e@o******a****s.org is IN the bounce list
Reason(s) : Recipient address rejected: unverified address: Address verification in progress
I already removed the address from our bounce list so you can receive emails now, I would also recommend you to check this guide in order to avoid bouncing issues: https://www.jotform.com/help/208-How-to-Prevent-Email-Bouncing-Related-Issues
If you keep experiencing the issue, you could ask your email provider to whitelist our IP addresses, the info can be found here: https://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses
