I am trying to add a WIDGET - Cincopa DeepUploader and it needs an API TOKEN. Where do I get that?

You need to log in to your Cincopa account: https://www.cincopa.com/login.aspx
Then, go to the API Tokens section to create the new key: https://www.cincopa.com/cincopamanager/api

I want to add a video recorder or video uploader to my form.

If the default API token does not work, then create a new one. Put it in your widgets general setup, where it says "API TOKEN", and then click on the "Update widget" button:
So that's it, the widget will be ready to start uploading files that will be submitted to your Cincopa account:

I am trying to add a field to a form that will allow the client to upload their own video file through the form.
What widget will do that?

In that case, you do not need a widget, you can use a native File Upload field, please check this guide to learn about upload forms: https://www.jotform.com/help/332-How-to-Create-File-Upload-Forms

That uploaded won’t accept .mp4.
Any other ideas?

Have you included that file type in the allowed list? https://www.jotform.com/help/29-How-to-Change-the-Allowed-File-Types-in-the-Upload-Field
