Why isn't my calculation updating the required filed?

I have two conditions:

I have a form submission where the passport type is ADULT, but the fields  {Christian name(s)}{Surname} have not been updated in the destination field "Submited by"

Please help.

I see two problems there, first you used the fields as numbers and not as strings, here is how to change that:

So you just click the fields.

The another problem is that you're copying two fields, name and surname to one field "Submitted by". 

So you should either have another field that is like "Submitted by" (so full name field) for your name and surname, or you should break "Submitted by" into two separate fields.
