    Hello,

    If you want to add a tab your own facebook profile this is the cause of the problem. Facebook profile page and Facebook page are not the same. A Facebook Page is a business account that represents a company or organization and a Facebook Profile is a personal account on Facebook. Adding a tab on your profile is not allowed, you can share your form on your profile by clicking More>Publish>Share Form>Facebook.

    I have just checked Facebook Tab and added to facebook page, it works fine.
