Form not loading properly

    Hello, 

    When I try to open link, link does not open it stays uploading but never shows the form.

    I cloned your form and I was able to replicate the issue. I checked the conditions and there are lots of error. I noticed that there are lots of missing fields on your condition. Could you please try finishing the condition settings?

    I removed all the conditions on my cloned form and the form is working properly.
