Trying to figure out if we have an account with Jotform

    There is a form being used on our website (www.cheetahstand.com) that is hosted by Jotform.  I am trying to setup a new form, but I have no idea what our username/password is for the existing Jotform account.  The existing form was created by another person, and I have no way to contact them.  All I have is the link to the form.

    Is there any way you can give me at least the username/email address associated with the form?

    Any help you can provide would be greatly appreciated.

    I have checked the form provided and see that it is associated with an account cheetahstandrma. This account is registered with an email address i***o@ch**********nd.*et. If you have forgotten the password, you may request for a password reset from the login screen.
