I am a consumer and can't seem to submit my form

    Im trying to submit a contact hour application to the Virginia Nuses' Ass.  I completed the required information, uploaded the file and hit submit....I get a Please wait.....it seems to be frozen.  Help!

    I did a test submission on the form (https://form.jotform.com/53165131087148) but I was able to submit the form properly.

    Can you please try again to submit the form again and see you will be able to submit the form properly?
