Thank you for contacting us.
Embedded forms are supposed to be responsive too. Please provide us with a link to your published page, so we will be able to check the issue.

You're correct, I checked the form direct link and it is working fine on my mobile device. But, if I open the Page URL that you have provided then the form is not displaying properly. Here's a screenshot when I checked the website URL:
Please re-embed the form using the iFrame Method. To do that, please follow this guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code
You can also check this screen animation for a quick guide:
I hope that helps. If you need further assistance, please let us know.

I did test your form embedded in webpage and I am able to replicate the issue you are having. It seems you have added custom css code in your webpage. Please check the screenshot below:
I would suggest you to please remove the existing custom css code from your webpage and add the following code:
Do get back to us if the issue persists.

If I add source code then its working. But but if multiple people opening in mobile after some time all the alignment is going off and all elements showing in a form..

Would you mind embedding with Source code and keeping it that way so we can test?
I tested the URL now, and it is showing the same.

I embedded code. as I said before its working now. But I don't know how safe it is.
I have to check in the morning if multiple people opened in the browser all the alignment is off.
all the elements values are showing on the form with overlapping.

Please do update us if you encounter any problems. So far I have tested your website on the following devices:
iPhone 6S Plus Safari browser:
Samsung Galaxy S5 Chrome browser:
Google Pixel Firefox browser:
I also used the Chrome browser dev tools to check the responsiveness of it.
Let us know if you are still having problems.

I hope I'm not hijacking this thread. It seems godavarifresh issue was resolved by a code change. I'm having the same issue but I have not touch any-code other than embed the Script code per the instruction (for Shopify).
It's responsive on all my browsers on my PC but not on my Iphone or Android phones.

If I directly to the form it works fine:
