Using forms with pre-filled information

for example for a class select the child’s name and then their personal information is added to the relevant filed on the form

We do have a widget that can pre-populate fields based on the uploaded spreadsheet. The widget is called "Spreadsheet to Form". Here's a guide on how to use it: How-to-Use-the-Spreadsheet-to-Form-Widget.
