I am unable to login and I have changed my email address

    I can't login, and I've changed my email from a.maxim@yahoo.com to amanda.maxim@aim.com. Please send me login reset instructions. I need to get to my forms. They are not going to the email I set up.

    Please note that the form you kindly pointed us to, belongs to account username AmandaMaxim and the associated email address with this account is neither a.maxim@yahoo.com nor amanda.maxim@aim.com which may explain why you were unable to login or receive password reset instructions email.
