Why are my users getting secure connection failed message when trying to submit the form?

I have had 2 separate users report the same issue 2 days in a row. Both users are in rural areas of the state and I wonder if it doesn't have to do with their connection and provider. They are probably still using dial-up.

Is there something that I can do in the settings of my form to make it simpler for them? We have to have required fields and this is preventing them from printing the form out and completing it first and then using that as their guide while they complete the online form.

I have been testing your form and noticed you are redirecting to a custom thank you page. The error your users are getting may be related to this redirection you are doing since your custom thank you page is not under a secure protocol (HTTPS).

The form is currently loaded under https://form.jotform.us/61405185606150 which loads under the HTTPS protocol while it redirects to http://www.squaremeals.org/

You would need to either switch your custom thank you page to a secure protocol or use the default thank you page.

This guide will help you to customize it: https://www.jotform.com/help/211-Setting-up-the-Thank-You-Page

    I hope this helps.
