How do I add a jotform to a magento product page short description

I want to add  a jotform to a magento product page short  and regular description. How do I place it in there as a link to the jot form?

Just take your form URL, follow this guide https://www.jotform.com/help/401-Where-to-Find-My-Form-URL and then paste it in your product page description. If the description box supports basic HTML formatting like hyperlinks, then it should be easy.
