Unable to Match Fields in MailChimp Integration wizard

Hi, I'm following these instructions for a Mailchimp integration: https://www.jotform.com/help/163-How-to-Integrate-with-MailChimp

I am not able to complete the Match Your Fields section, since the fields do not show up (see screenshot). I have tried starting a new form with a new API key but keep running into the same issue (per the advice here: https://www.jotform.com/answers/722623-API-Mailchimp-Why-I-can-t-match-my-fields). Do you know what is causing this?
