Do you have a sharing form?

I'd like to set up a form that allows visitors to my site to recommend the site via email to five of their friends. It would then notify me that they've done so (and send me the info the user has provided so I can send a small premium as a thank-you).

Is that something that exists among your templates? I've not been able to find it.

You can create a form that will ask the visitor of your website for 5 email addresses (referrals). Once the form is submitted an email will be sent to the 5 email addresses in a form of an autoresponder — Setting-Up-an-Autoresponder-Email

You will be able to construct/design the content of the email autoresponder.

So you will be notified about the referral, you can setup an email notification in your form so that every time the form is submitted successfully, you will receive an email — Setting-Up-Email-Notifications

You can also ask for the email address of the visitors of your website on the form so you will know where you can send the premium showing you appreciate what they have done.
