Why has my account been suspended?

    Just wondering Why has my account been suspended?

    I have checked your account and it has been suspended due to the bank account fields you have on your form http://www.jotform.co/form/71446074340855, here is a screenshot of those fields: 

    Please do note that asking for such information is not allowed on JotForm and although you are not using your account for phishing purposes we need to act according to our terms.

    I have already deleted the fields from your account in order to reactivate your account. We can do exemptions and allow accounts to ask for such information, these are the exemptions: 

    - If you send a proof that your business is related to : MORTGAGE BROKER, INSURANCE BROKER BANK, Real Estate Broker, LENDING INSTITUTIONS and the like. 

    In case your business is in the mentioned legal businesses above, you will be able to keep asking for such information,  but a signature field must be added, so your clients will be signing to be in agreement with providing such information.

    We would also recommend you to upgrade your account, this way your account is manually reviewed by our anti-phishing system and you will avoid to be suspended, while if you have a free account your account will be automatically suspended without prior notice.

    Thank you for the information provided. That makes sense to ensure security.
    I wish to request an exemption based on the fact that I am a *registered
    BAS Agent / Bookkeeper*.
    Please search this register for the *Australian Tax Practioners Board* for
    my Agent # and you will find my business listed.
    https://www.tpb.gov.au/search-register
    My form already includes a signature field, as suggested.
    Please confirm the form is now OK to send out to my clients.
