So i am trying to create a JOT FORM, with name "Refund Form".
Here is the scenario - this form has 2 pages. On page 1 there are details for the refund request and when a person submits the form (on page 1) - notification is send out to us for "Approval" (which is on page 2)
I was following the thread to make my approval work flow.
In notification i have added the link as mentioned on the above thread - however when i click on it - it gives a 404 error.
Quick help or response on this will be much appreciated.

I tested a clone version of your form and without making any changes to it, it seems to have worked perfectly. I have also checked the submissions made on your form and the link that was produced on the email logs from the submission. Perhaps you were able to fix the issue on your end. Otherwise, please feel free to let us know if you need any further assistance.

Yes - Thanks for getting back to me. I was able to figure it out. But i do need an assistance with this form. Let me give you a picture of this form and what we're trying to achieve out of this.
1- Request is submission from page 1. [Done - working fine]
2- Email notification is sent out to me and Person A - [Done - working fine]
3- Person A gets a link to approve this request: [Done - working fine]
4- Person A follows the link and lands on page 2 - approve or declines the request [Done - working fine]
5- Email notification is sent out to me and Person B [Cannot figure it out - need your help]
So basically on submission of form for second time (approval or decline) a different email notification should be sent out and it should be sent out to different people.

Update on above comment or request - i was able to figure that out as well.
The last thing which i see as a problem that - on page 2 i have hidden an email address and when ever i fill out the form - its not hidden its showing up.

Yes i did - i had to show it to management for review. If you have a clone of this form you can try to see what i meant. If it works great i'll put it on the form as per guidance.

I have cloned your form named "Refund Form" in order to test and investigate it.
I have seen the Accepted/Declined part of the form in the second phase when the notification comes. When I have accepted/declined the approval via submission edit link, I have also received an information email as seen the example below:
I have seen that the form works successfully without any issue. I do not see any problem currently.
