hi, I am trying to log into my account which now shows as suspended. I've tried to change the password which saved but still no luck.
please advise as to what the problem could be.

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
In order to activate your account, it is required to agree to our terms and to delete banking fields.
Please let us know if you agree with our terms and we will delete the fields and activate your account.

hi, thanks for this. I had no idea. I agree with your terms and conditions and that you can delete these fields.
I presume I can use the SSL and Encrypted Forms feature easy so I can then still collect this information?

Actually, it's not about the encryption or the security of your forms that is the concern, but it's the practice of collection of information. So I would suggest other means of asking for these information.
For now, I have reactivated your account. I suggest to remove the fields otherwise there is possibility for the forms to get flagged by the system for suspension again.
