Getting an "Import Failed" error message when trying to import a PDF

    Im trying to import a pdf and keep getting this email message: 

    Oops, Import Failed!

     

    Unfortunately, we were not able
    to complete your PDF Form.

     

    Please help me get this pdf uploaded. Thanks.

    We would like to apologize for any inconvenience this is causing you. Are you trying to import a PDF form from FromCentral to Jotform through this page — https://www.jotform.com/adobe-formscentral-alternative/ following this guide — How-to-Import-Adobe-FormsCentral-PDF-Forms-to-JotForm

    Or, are you uploading your PDF form through this site — https://www.jotform.com/fillable-pdf-form-creator/

    If the PDF form you are trying to upload is not from FormCentral, then I suggest you use the latter.

    However, if you already did and you are getting the error message you described in your post, please let us know and please share with us the PDF Form you are trying to import. (You can upload it to any file hosting services like Google Drive, OneDrive, or Dropbox and post the share link here in this thread. Please make sure that the share link is public)

    Hi Mike, I tried it both ways and neither worked. The pdf is below:

    I would like to apologize for any inconvenience. I have downloaded the PDF form you shared with us and tried to import it and I am able to reproduce the issue you are having.

    I will have this concern escalated to our development team so they can have a closer look at the issue. Although we cannot provide any ETA to when this will be fixed, please be assured that once there is an update from our development team, we will notify you in this thread the soonest.

    Also, while waiting for the issue to be fixed, you can also try to just recreate the form here in Jotform. You can use the Narrative Fields widget to make it look like the same as the PDF form you are trying to import. Please let us know if you need assistance doing this and we will be glad to help you.

    I created the from the way you suggested. It is at the link below. When filled out however, the email I receive looks nothing like the form that was filled out. I need to receive the email of the filled out form looking like the one that was filled out. How can I do this?
