    Hi, 

    Can you please help me get my account unsuspended, or at least tell me why it was suspended.

    We have several websites that use jotforms.

    It looks like one of your very old forms requests log in credentials.  It is your Automated Payment Signup Form.  I am not sure why it is causing trouble now, but collecting usernames and passwords through forms is against our terms of use.

    This form has no submissions and was created December 2013.  If you wouldn't mind us removing the form, we can activate your account right after.

    I deleted the form and activated your account.  Your forms should be usable once again.
