Having problems getting any email from this form. It worked perfectly well and then just quit working. I have tried numerous different email addresses and nothing goes throughPage URL:

There are two issues here:
1) There is no Notifier created,
2) Autoresponder is not set to go to email address field.
So you should create Notifier and set Autoresponder to go to email field:
