What link is my jot form enquiry template connected to?

I have recently upgraded my shopify website. Since the upgrade I cannot see jot form on my website anywhere however my customers are finding the jot form enquiry template and sending enquiries through it. 

Im very confused. 

I have checked your webpage and I was unable to find any embedded JotForm, but there seems to be only one form on your account which  should help us to know where the submissions are coming from. 

I would recommend you to edit your form in the Form Builder and add the "Get Form Page URL" widget to your form, this will hep you to know the webpage  where the form was embedded on when it was submitted.

Once you get the page where the form is currently embedded  on then you could check the source code  and either remove the form or update it.
