Is there a way to have one person fill out the form and have it sent to a second person for a signature, then sent to third person?

I am trying to create a reimbursement form for our parent group.  The president would like to have the parents fill out the form and attach receipts (have that form made already) then send it to her.  She would then review the request and electronically sign it and it gets sent to the treasurer?
