I am photographing church directories and need to make these appointments available for only certain dates. How do I do that? Also, the page name that appears on the tab when I click on the icon to go to the page has the name of the sample I used to create it. It says "NJ Boxer Mini Sessions" and I need for it to say "Directory Sessions" How do I do that? Thanks.

Are you looking for a way to disable your form and limit submissions based on certain dates?
If so, you can refer to this guide — How-to-Limit-Submissions-in-a-Form
Or, are you looking for a way to just disable fields based on certain dates?
You can add a Date Picker field on your form and set it to Current
then conditionally disable fields in your form based on the value of the Date Picker field following the instruction in this guide — How-to-Enable-Disable-Fields-Using-Conditional-Logic
