Issue with conditional logic not result to expected answer?

    PAGE 4

    Form Field "Employee Responsibility" conditions work GREAT!

    PAGE 5

    Form Field "Employee Responsibility" conditions are EXACTLY the same as PAGE 4 and do not work.

    Can you help me, please?
