Used to work then all of a sudden stopped. The notification works fine but the autoresponder does not appear to work corectly.
Noticed that I am only able to select "email@example.com" as a recipient, whcih I presume is at least part of the problem.

Your autoresponder does not have a Recipient E-mail configured. You must select the email field to have it sent to your visitors.
