Why is my form title reading "clone of" on my google tab?

The form title is "2017-18 Winter Work Form" and reads as such.  When I go to the link to fill out the form, in chrome, the description of the page I'm on says "Clone of 2017..."

It is a cloned form, but I've deleted that default description.  Also, I have another form that I did the same thing with and the description in the tab does not read "Clone of..."
