Hi, I used one of the templates because I wanted to include my logo at the top. I made my form with a bunch of questions, saved it and emailed it to myself, but half of the fields are missing. Can this be fixed?

If you have edited your Email Notification previously (before editing your form), you will need to edit the notification again. by clicking on the elements on the right panel of the Email alerts editor the fields that are not in the content of your email alert.
The fastest approach on this is to simply delete the old Notificaiton and create a new one.
