I need to set-up a form that would allow a family to pay one registration fee with multiple registrations. I would like for each member of the family to have a separate 'row' in the excel report. Is this possible?

One possible solution might be to use the Matrix field, so they can fill the names of each person in the family. But, it will not show each person's name on a separate row on the Excel report.

Here is an example of multiple guests registering for an event:
This form uses the "conditions" feature that is found on the "set-up & embed" tab when you are editing your form. You can use this feature to only show the fields for the 2nd guess, 3rd guess, etc only when the user chooses to register mre then one person.
As per the Excell report, I'll take a look to see if I can manage that. Hope it helps!
