Lightbox form is not loading fully on my site, just opening a blank screen

The form can be found at www.learnirl.com/teachaclass. Clicking the "Sign Up To Teach" button should open the lightbox. It worked for about ten minutes. Now it will not load...

I checked your site (https://learnirl.com/teachaclass) but I wasn't able to replicate the issue on my end. The light box with the form fully loads for about 4sec to 6 sec.
