Login failed - Suspended

    Why was my account suspended? How do I reactivate it?

    The form "Opt In Form #2" is asking for login information which violates our terms of use. That's the reason why your account got suspended. As per our terms, we do not allow collecting login information here in JotForm. Also, the word "Password" will get your account automatically suspended because of our anti-phishing detector tool. 

    We need to delete the guilty fields or the form so we can re-activate your account. If that is okay with you, let us know so we can proceed.

    Please refrain from creating such forms or adding any guilty fields to the forms to avoid any further suspensions on your account.
